A Guide to How We Collect, Protect, and Share Member Information
To provide you with high quality products and services, the American Fraternal Union must collect a certain amount of personal information about you. We also may share limited information about you with organizations we’ve formed relationships with to provide you better services. We do not, however, sell or rent our member’s personal information to other business concerns.
Your privacy is very important to us and that’s why we have established policies and safeguards to protect the personal information that we collect from you. This notice summarizes our privacy policy as it pertains to our members and prospective members.
Information the American Fraternal Union Collects and Discloses
We may collect nonpublic personal information about you. This includes:

forms, such as your name, address, social security number, and personal

hospital records. This medical information will not be disclosed by the
American Fraternal Union without your authorization, except as required by

contract coverage, investment and premium history. | |

This information will be shared only for appropriate situations and only to the extent required to carry out your business with the American Fraternal Union.
We may disclose the above categories of nonpublic personal information to nonaffiliated business partners in order to complete necessary business transactions on your applications, account, contract, or in order to comply with legal requirements such as subpoenas or tax or fraud reporting.
We may also disclose this information to nonaffiliated business partners that perform business services on our behalf.
We will not disclose your contract or account numbers to any nonaffiliated business partners except where necessary to complete business transactions.
How the American Fraternal Union Safeguards Your Information
The American Fraternal Union is committed to protecting the personal information we have about you. We have instituted extensive internal controls to safeguard your personal information, which include:

In order to better serve our membership, we may disclose limited personal information about you to organizations that we have formed relationships with. This limited information may include membership status, name and address, demographics, and financial categories. No specific information about your finances will be disclosed.
You can choose to not have your information shared with other organizations. The American Fraternal Union is providing you with two options:
| Allow the American Fraternal Union to share information with
affiliated organizations. If you choose this option, no action is required of
you. | |
| Opt-out of sharing information with affiliated organizations. If you choose this option, the American Fraternal Union will not share information about you with other organizations. If you choose this opt-out option, please call 1-800-346-1289 and advise us of your decision. |
The American Fraternal Union reserves the right to change our privacy policy and practices, but if we do we will provide a written notice to you.
